I. DOCUMENTARY REQUIREMENTS TO FILE A CLAIM
A. Own Damage
Pertains to accidental damage to the insured vehicle
1. Policy
2. Policy Report or a Notarized Affidavit executed by the assured
or the authorized driver thereof
3. Photocopy of the Certificate of Registration and Official Receipt
of the insured unit
4. Photocopy of the Driver's License and Official Receipt of whoever
was driving when the accident occurred
5. Pictures of the damage sustained by the unit showing the damage
as well as the whole car with the license plate visible.
6. Repair Estimate
B. Compulsory Third Party Liability
Pertains to bodily injury resulting from an accident by an insured unit
covered under the Mandatory CTPL coverage
1. Policy
2. Policy Report or a Notarized Affidavit executed by the assured
or the authorized driver thereof
3. Photocopy of the Certificate of Registration and Official Receipt
of the insured unit
4. Photocopy of the Driver's License and Official Receipt of whoever
was driving when the accident occurred
5. Medical Certificate of the victim
6. Any and all medical and hospital receipts in relation to the
victim's injuries
The NO FAULT provision under this coverage offers a maximum
indemnity to a victim in the amount no more than Php 10,000.00 as
provided for in the policy whether or not the authorized driver is found to
be at fault or not.
C. Excess Bodily Injury
This coverage refers to the voluntary coverage obtained by the
assured
specifically for Bodily Injury Claims, which will only respond in excess of
the CTPL coverage. Documentary requirements and claim procedures
are the same with that of CTPL.
D. Third Party Property Damage
A TPPD claim is filed when the insured unit causes damage to a third
party property. Documentary requirements may vary with respect to the
nature of the property damaged.

1. Certificate of Registration and Official Receipt
2. Driver's License and Official Receipt
3. Police Report
Third Party documents - in case this is another vehicle involved:
1. Certificate of Registration and Official Receipt
2. Driver's License and Official Receipt
3. Pictures
4. Repair Estimates
5. Certificate of No Claim
II. CLAIMS SECTION
A. Receiving Section
1. Claimants may submit their documents to the receiving section
located at the lobby of the Claims Department. All documents will
be stamped with our receiving date and duly signed by the
receiving staff.
2. A claim number will be assigned by the claims computer system
upon encoding of all documents submitted by the assured.
3. All submitted documents are passed on to the processing section
for
evaluation of the claim.
B. Processing Section
All submitted claim documents are given to the claims examiner for
evaluation and consequently for the issuance of a Letter of Authority or
Cash Settlement Offer.
III. ACCREDITED SHOPS
Should the client bring the insured unit to any of the accredited shops of
Prudential Guarantee listed below, the said shops will take the responsibility of
assisting the client with regards to the documents and the repair of the vehicle.
