Submit the following documents to Prudential Guarantee:

1. Filled-out Notification of Claim Form or a Medical Certificate stating the dates of confinement, surgical procedure done (if any), diagnosis and signature of the attending Physician.
2. Original Official Receipt of:
         •  Hospital Bill
         •  Professional Fees
         •  Medicines or Lab Test bought or done outside of the hospital
3. Itemized statement of account of the hospital
